Open Integrations
In the form, go to the Integrations tab and pick Slack.

Authorise your workspace
Sign in to Slack and allow Onlyform to post messages. Only the channels you choose ever receive anything.

Pick a channel and the fields
Choose where messages go, tick the answers to include, and switch it on.

Messages are not arriving
Check the connection is still authorised and that the Onlyform app is a member of the channel. Private channels need an explicit invite.
Can I post to more than one channel?
Yes. Add the Slack integration twice on the same form with different channels and conditions.
Can I customise the message?
You choose which answers appear and in what order; the layout itself is fixed so messages stay readable on mobile.
